This Hyperballad promo was released by Elektra Records in the US in (probably) 1996. There is no artwork, just a sticker on a white record sleeve (shown above) and the generic Elektra label on the discs themselves. This is the double 12″ single for Big Time Sensuality released in the US in 1993 by Elektra Records. I purchased it brand new from eBay late last year.
